1. Please print and complete the Preschool Application Form (all three pages, front and back).

2. A non-refundable $50 application fee is due at the time of registration. Please make checks payable to: “Cornerstone Berean Church” or “CBC” and in the memo write: “LBP Application Fee”.
3. Mail the completed application form and your check to:

4. You will receive an acceptance letter in the mail to confirm your preschool registration and preschool session preference.
